Summary of talk:

HeartCare at Home is a home monitoring programme for patients with heart failure, delivered by Centric Health in collaboration with Roche Diagnostics and the HSE Digital Transformation team. Current estimates suggest over 90,000 people in Ireland have heart failure with a further 160,000 at imminent risk of developing heart failure. This programme provides remote management with direct clinical support for patients to self-manage their heart failure symptoms at home.

The programme ultimately aims to facilitate collaborative and integrative patient centred care between the patient, their GP and their Heart Failure team while supporting and encouraging self-management of their heart failure in a safe yet innovative manner.
